Child Care Providers,
The Iowa Department of Public Health, Bureau of Family Health requests your organization's participation in a survey on child and adolescent health services in the state of Iowa. Your responses to this survey will inform public health planning to allocate public health resources across Iowa's 99 counties. The survey is being conducted by a research team from the University of Iowa College of Public Health.
The survey requests information about your organization's provision of early childhood developmental screening services. All survey responses will be included in a final report on the availability of these services in each county. The purpose is to assess for gaps, assure access, and avoid duplication of services.
On January 4th, 2021 an initial round of survey invitations were sent out via email. Each email contained a unique link to the survey.
